Signing up with the Endocrine Care Group for Diabetes Management or Second Opinion Consultation is an easy process and can be completed in 4 steps. We have outlined the process below for both services:

Step 1: Click on the desired service (Diabetes Management or Second Opinion Consult) and select the ENROLL option from the right hand corner of the page. If you select Diabetes Management, you will also need to select a specific Diabetes Management Service in terms of duration and/or Pump regulation program. 

Step 2: Registration and Payment

Step 3: Share with us your medical history by completing a series of online forms.   We will also send you a Disclaimer form and Primary Care Provider referral form (if necessary). These forms can be returned via e-mail (endodocs AT endocrinehelp.com), fax (843-795-4209) or mail.  We also request that you mail, email or fax copies of laboratory reports, pertinent radiology studies and physician notes to our office

Step 4: Upon receipt of your request, medical information and payment, we will send you a confirmation notice. If there are missing documents, we will notify you as soon as possible. We will arrange an initial appointment by live video, telephone, or instant messaging, after which our initial medical assessment /recommendations will follow within 48-72 hours.

 
For individuals signing up for the Diabetes Management program, glucose log sheets will be made available to you throughout the process, which can then be returned to us by e-mail. Alternatively, you can simply write your blood sugar log in the body of an e-mail addressed to the Endocrine Care Group. We will also accept EXCEL files, scanned copies of Glucose logs or Glucometer readings downloaded from other software.

It’s that simple!

Be assured that your medical information will be treated with the highest levels of privacy protection. Copies that you provide will be kept for a maximum of 6 months, then destroyed, or can be disposed of earlier at your request. Our assessment reports will be kept on our files.
