Who / What is the Endocrine Care Group? The Endocrine Care Group is an online medical practice, staffed by Board Certified specialists in Endocrinology. You can think of it as any other medical practice that has a physical office that you visit, in that there are real people answering your questions and attending to your concerns. The only difference is that a physical exam cannot be performed. How can an on-line medical practice help treat or diagnose me? Do I need to be seen in person? There are several medical conditions that may be treated and even diagnosed without a physical exam, and many of these diseases are found within the field of Endocrinology. The combination of your personal information including current history of your complaints and symptoms, your laboratory tests and any radiology studies, can often provide the necessary information for a diagnosis to be made, even without a physical exam being performed. If your diagnosis/concern requires a face-to-face examination, we will make this recommendation to you as early in the consultation process as possible. Because a physical examination should be performed at some time during your evaluation, we strongly encourage you to stay in contact with your primary care provider who will be able to perform necessary examinations or refer you to another specialist who can do so optimally. What kind of problems can the Endocrine Care Group help diagnose or manage/treat? There is a wide range of endocrine/hormone diseases and problems that we will be able to assist in diagnosis or management, including the following: Diabetes types 1 and 2(insulin requiring and non-insulin requiring), insulin resistance syndrome, thyroid disorders, adrenal problems, deficiencies in various hormones, pituitary disorders, reproductive problems (including erectile dysfunction and abnormal menstrual cycles), excess body hair, evaluation of fatigue and weight gain, nutritional deficiencies, osteoporosis, cholesterol problems, high blood pressure and rare Endocrine diseases such as MEN syndromes. This list is not exhaustive. Please note that there are variations of some disorders that may best be served by a face-to-face evaluation by a specialist. Are there some Endocrine disorders unsuitable for consultation with the Endocrine care Group? Yes, and we will alert you to this very early in the process if you have already signed up for our services and give a full or partial refund of your fees. An example of such a concern that cannot effectively be managed by our online practice is a thyroid lump or nodule unaccompanied by radiology readings or not previously examined by a provider. How do I sign up for services? After reviewing the information about our different services, you select your chosen option at the upper right hand corner of the screen: Diabetes Management or Second Opinion”. You will then be taken to another screen where you will chose a selection within those services, for instance, choosing a one-time consultation for diabetes, or ongoing consultation with follow up for 6 months duration. The screens will then direct you through the check-out process and you're done! How soon will I get a response? After you have completed the registration process and selected a service(Diabetes Management or Second Opinion), you will be contacted within 24 hours by the Endocrine Care Group by e-mail to confirm your request. After we have received all the necessary medical history and laboratory information from you, we will send a formal recommendation within 24-72 hours. If you are enrolled in an ongoing Diabetes management program, you will receive feedback on your submitted glucose logs within 24-36 hours. Can I sign up for both Diabetes management and a Second Opinion for another concern? Yes, we certainly can address both issues. You will need to register and sign up for both services, and send all pertinent information regarding your potential diagnoses. We will communicate with you in regard to both. What is the difference between a one-time Diabetes consultation versus the Ongoing consultation? Our one-time opinion will involve review of your medical information and one set of glucose data, after which we will render an opinion in regard to your current diabetes management and any recommendations for changing your treatments, as well as for future follow up. The Ongoing consultation will include recommendations after review of your information, but also includes review of unlimited numbers of glucose logs submitted by you during your selected time period. Given the chronic nature of diabetes we suggest our ongoing consultation as more optimal for your care, even if you chose to do so for only a few months. How do I submit records or reports to the Endocrine Care Group? Copies of laboratory studies, radiology reports and provider notes can be faxed, mailed or e-mailed as scanned attachments to the Endocrine Care Group. Glucose logs can be sent similarly, and also written in the body of an e-mail. We also accept EXCEL files and computer downloads of your logs if you are using other software, including diabetes management devices such as online programs or real time feedback systems (for example CONFIDANT system). We send confirmatory e-mails every time we receive any information from you so that you are assured your information has been received and is secure. Will my information be secure? The Endocrine Care Group utilizes a secure web page for all transactions and forms that contain any medical information. Hard copies of medical information that are sent to our office will be kept in locked files for 6 months, thereafter they will be destroyed. Access to all information is limited to the physicians of the Endocrine Care Group and the administrative webmaster. Be assured that your information and transactions will be handled with care to preserve privacy in accordance with HIPAA guidelines. |
