Welcome to our Intensive Diabetes Management program!

This program was designed to help you gain better control of your diabetes, by increasing your understanding of the treatments available and individual tailoring of your treatments to fit your lifestyle and goals. Diabetes affects people in different ways, making a one-size-fits-all treatment plan often unsuccessful.  Our program is tailored to your needs and changing diabetes status to make this YOUR treatment plan.

Your 3 Phase Program

Phase 1

  • An initial consultation during which we learn about your history and lifestyle. 

Phase 2

  • A review of your diabetes management in the form of questionnaires and recent glucose level recordings/logs.
  • Design and establishment of your treatment plan.
  • Define goals for your diabetes management.  

Phase 3

  • A review of your goals at the end of the program.
  • Recommendations for your continued care.

Program Options

Initial Consultation and One-time Feedback Only—
You share your history and current treatment as well as recent glucose logs. After a thorough review of the information, we provide a one-time recommendation regarding your current treatment and future course of care. Recommendations will be provided via e-mail, fax and/or mail.  This option can be used for diet controlled diabetes, those on oral medications, and/or insulin (injections or pump).

Initial Consultation and Individual Follow-Up Programs—
You share your history and current treatment as well as recent glucose logs. After a thorough review, we provide recommendations regarding your current treatment plan and course of care. Throughout a specified time period varying from 2 months to 12 months, you submit an unlimited number of glucose logs, and we will provide feedback regarding recommendations for your treatment plan/medication changes, as well as general diabetes care and education.

Insulin Pump Regulation Program—
After sharing your history and current treatment plan as well as glucose logs, we provide a thorough review and make recommendations regarding changes to basal rates, carbohydrate ratios, basal rate checks and correction doses.  Your program duration can be anywhere from a 2-12 month period of time.  If you desire instead a one-time consultation, you should select the Initial Consultation and One-time Feedback Only opition listed above.

What You Can Expect

Review of your information and recommendations by a physician who is Board Certified in Endocrinology and Metabolism. Diabetes management makes up the largest component of our professional training and work, and we are always up to date on the latest treatments and recommendations for your care.

A timely response. After receipt of your information, by e-mail, fax or mail, we will contact you within 48-72 hours to review and/or make treatment recommendations.

Professional and secure interactions. We do not provide any information to other physicians, laboratories, or any individual unless a written request is provided and signed by you. Your information is collected and stored in a system that is compliant with government guidelines  (HIPAA). In addition, our opinions are unbiased by any association with other parties such as pharmaceutical or laboratory companies.  We are not affiliated with any companies that sell or produce Diabetes medications or products.

Written instructions by email, fax and or mail each time an opinion is delivered to you. We will also provide a copy of these recommendations to your Primary Physician if you request, or if you are referred by your Physician.

What Not to Expect

Solicitations of any kind. This is a medical practice and our goal is to provide service for your diabetes care. We do not sell any products.

Frequent phone calls to your home. We conduct most of our communication via e-mail or fax, or can do so by mail if requested. This allows you to review your recommendations when convenient for you.

Prescriptions or laboratory requests. While we will certainly make recommendations regarding medication dose changes, medication changes and laboratory investigations, we prefer to leave the prescribing in the hands of your personal Physician, who will be following up with you in the future.  While we do not require it, we suggest that you notify your personal physician when you agree to consult with us, so that we can involve them in your care as well, thus allowing for smooth continuation of your care in future.

We are licensed to provide diabetes management for people from California, Florida, and South Carolina.  We can also receive referrals from your primary care physicians if you are from other select states.  Please contact us for additional information.  

Your Important Role

Diabetes is one of the few diseases in which you can exert some control, and in many instances, a great deal of control, over its course, severity and outcomes. Your role as an involved and educated person makes all the difference. In Intensive Diabetes Management this means:

  • Providing us with accurate and up-to-date information, regarding your health history, various measurements such as weight and blood pressure, the most recent laboratory studies and medications.
  • Being upfront about changes that you make on your own to your medication regimen or eating habits. 
  • Keeping and providing accurate glucose and food logs.
  • Asking questions about your health, your medications and alternative treatments. 
  • Responding to questions/queries in a timely manner.
  • Giving your full effort towards improving your health. This may involve being open to other suggestions for lifestyle changes and breaking old habits.

 
Your Primary Doctor's Role

While not required, we do encourage you to notify your Primary Physician of your consultation of our services. In some instances, your Physician may have suggested our program to you. In either case, your physician plays a very important role in

  • Following your progress throughout the program
  • Prescribing new medications or supplies that we may suggest to you.
  • Ordering laboratory studies necessary to track your progress and treatment parameters/goals.
  • Responding to emergency questions regarding your health.
  • referring you to other specialists if necessary.
  • Continuing your care after the period of consultation with the Endocrine Care Group has concluded.
